Amending a project

What is an amendment?

An amendment is a variance to the original approved project, including changes to research personnel. Some modifications may be so significant that a completely new application is required. Contact the Human Ethics office if you are unsure whether an amendment request is appropriate.

1. Create amendment request

A request for amendment is created in Themis. The Quick Reference Card below gives detailed instructions.

2. Print a hardcopy

A signed hardcopy of the Request for Amendment is also required.
Before submitting also attach any other documents that have been revised as a result of the amendment (e.g. Plain Language Statement, consent form, questionnaire).

3. Who do I send it to?

If the amendment is associated with a Minimal Risk project or a Project-Within-Program, and presents no additional risks, then it can be considered and approved by the HEAG. All other amendments should be submitted initially to your HEAG for endorsement, and then forwarded to the relevant HESC for approval.

4. When do I send it?

Amendments to HEAG-approved projects can be submitted at any time. Those intended for review by a HESC should be submitted to coincide with the relevant meeting deadline. See meeting dates and application deadlines.

5. What happens next?

Keep a copy for yourself and await notification of the outcome of the HEAG or HESC review.
